Have the glow plugs been changed recently? If one is defective it can cause the car to sound like it's not firing on all pots and therefore you'll get some unburnt fuel coming out the tail.

I had a similar problem a few years ago, changed all the plugs and hey presto, she started on the twist of my key first time and minimal smoke.
