Something simple 
For my 21st Scott and I spent the day in London, shopping and taking out a pedalo on the Serpentine. It was so simple but really nice to do something together, that we prob ably wouldn't do normally. Depending on where you are, how about taking her to a show, or treating her to a day in a spa with you?
Why don't you make her a special gift? Something like a scrapbook of memories about the two of you, or a CD with all the songs that remind you of her/that mean something to you two as a couple?
Or you could go all out and just propose......
