Just thinking out loud, but is the high pressure feed screwed into the block the same as a normal feed? Have you cross-threaded it in the block? I'm just thinking out loud, but is it possible if it wasn't put into the block correctly, could it have popped out when the oil pressure was up?

was the turbo all ok BEFORE you fitted?? seals/bearings etc?

and anyway to ensure it is actually getting a good oil feed?
